Update: What Does the Future Hold for the PGR.....

Due to a lack of time on my part, and a lack of much interest in the PGR after well over a year of existence, I am withdrawing it from the blogosphere unless another brave soul is willing to take over. Please email me at clarkbar2019 AT yahoo DOT com if you are interested in the task. It's been fun at times, but it mostly felt like work. And I have moved on to another very time consuming project that just doesn't fit very well with the PGR.

Beginning with the upcoming September 2nd edition of the Pediatric Grand Rounds, future editions will be held on the first Sunday of every month. Therefore, each volume will contain twelve editions beginning with the third. To avoid any confusion with the numbering, the current volume will continue with edition 9 and will contain a total of 16 editions. "This has been a very sobering week. A prolific and very much admired blogger, Flea, was unmasked in a very public way during his malpractice trial. There is a lot of talk going around on the medical and law blogs about this, most people agree that blogging about an ongoing trial is not a great idea no matter how anonymous you think you are. To me the saddest part of the whole thing is that there are two grieving parents who have a dead son and there is a Dr. Robert Lindeman who could possibly loose his practice because he made a mistake (a definite blogging mistake and a possible medical mistake). It just does not seem fair to me that the lawyers are the only winners in this whole thing, they get to collect their paychecks and go on with life as usual.

The loss of Flea is a huge loss to the medical blogging community. He wrote out many evidence based posts on things that were being widely discussed about children's health. His series of posts on vaccinations are treasures that may never be replaced. His looks at alternative therapies and possible causes for Autism were also reoccurring themes in his blog. I especially loved how tried to empower parents by teaching them to take care of simple illnesses at home, keeping sick kids out of the doctor's office. I will never look at Neosporin the same way again.

Flea, you are gone and very much missed. Dr. Lindeman, I hope that you and your family are able to recover from this and hope that some day, some how we will be able to hear more from you.

As spring finally makes a return, the blog world in my small corner of the internet has been less than joyous. The lilacs may open with their lush fragrance, the hyacinth may reach yellow branches skyward, the tulips may bend gracefully at the end of their long stems, but still many of us resist nature's call to celebrate. The news has been rough and bitter, full of bombings and unanswered cell phones.

In spring, the weight of sorrow seems particularly unfair."

"On April 23, 2006, the first edition of the Pediatric Grand Rounds was published. And for the past 52 weeks, with a few notable exceptions, 25 subsequent editions have been posted every other Sunday. After a slow start, with just a handful of submissions, the PGR has grown considerably though it still pales in comparison to the behemoth that the original Grand Rounds has become since its inception. That is probably not a bad thing however. Many people have chosen to forgo reading Grand Rounds because of its size.

Putting together the past 12 months worth of PGR editions were 16 different bloggers with a variety of backgrounds. Some of them were pediatricians of both the general and specialist kind, nurses that have experience with young patients, or others who provide health care for children in less traditional, but no less important, ways. Some were involved in research with potential pediatric implications and some were parents of chronically ill children. Each approached their turn to host an edition in a unique way.

I've spent the past week returning to these prior editions, reading the posts, and choosing what I think were the cream of the crop. Yes I admit this was a quite subjective process with results no more meaningful than any other personal list of favorites. I tried to approach without any bias from my background as a pediatrician and I think I have compiled a pretty decent group of submissions. Please don't be hurt if you weren't included and feel free to add your own favorites in the comment section."

"It seems that many of the pediatricians are swamped with patient care duties. It could be the tail-end of RSV season, the onset of rotavirus season (depending on where you are), or the ongoing norovirus outbreaks. Whatever the case, this week’s call for grand round submission resulted in more entries from the recipients of pediatric care than from the providers. As always, because the purpose of grand rounds is to broaden one’s perspective, I’ve included equal number of both and also purposefully created the artificial divide of physicians and patients to allow one to compare and contrast the perspectives."

"Whether we've taken an elegaic turn because it's approaching the end of year, or there is a general zeitgeist, the paediatric blogosphere offers many thoughtful posts. We have sex, drugs, issues of life, death, morality; ethical dilemmas that reflect religious and cultural values as well as the challenges posed by advances in medicine. Because this is PGR, we also have snot, magical thinking and the general weirdness of contact with children.

Machiavelli famously claimed that statecraft involves fortifying the banks against the rivers of misfortune. We're not all the way there, but most of us are fortunate enough to live in societies where we have fortified the banks of paediatric health against the erosive forces of disease, morbidity and mortality. We live in good housing, we have clean water, standards for air quality, good nutrition, safety standards, improvements in medical science and public health measures such as vaccination. Most of us can be confident that children will survive through to adulthood."

Due to a minor and thankfully reversible tear in the fabric of spacetime, and the resulting existence of a previously unrealized reality, two seperate editions of the 18th edition of the PGR materialized on December 19th, 2006. There is as of yet no explanation for this anomaly.

"I'm a big fan of modern medicine. I mean, ever-lengthening life spans? Near eradication of many serious childhood diseases? Babies tiny enough to fit into the palm of a hand growing up to run amok on playgrounds? Movies-on-demand in patient rooms? These are good medical times.
But let's face it: there's nothing scarier than a dark hospital corridor at 2 a.m. Unless maybe it's a dark hospital boiler room, at any hour of the day. Even business-as-usual, broad daylight events in a hospital can scare the sweatpants of an unsuspecting parent. Take this single day we experienced on our last PICU stay with Annika:

9:36 a.m. A member of the hospital's new Central Line Team enters the room. We smile and wave, expecting the usual check of Anni's central line dressing. Instead we were startled to see her begin to unfold a surgical towel containing line placement supplies.
Us: Excuse me.
Her: (continuing her work) Yes?
Us: Are you getting ready to place another line?
Her: (beginning to check veins for a good site) Yes.
Us: I think there's been a mistake. She's not supposed to be getting any more lines.
Her: (finally stops work long enough to give us a quick glare, before changing to a voice dripping with condescension) Your child is very sick. Your child needs to get medicine. Sometimes the doctors and nurses need to have more places to give medicine. Especially when the child is very sick. It's important the nurses can give medicines when they are needed. Because your child is very sick.
Us: (a few blinks, wondering what to make of this treatment) Um, yes. But the latest plan was to consolidate meds and pull as many lines as possible, given that she's having Mystery Fevers. To eliminate possible sources of infection. (finishing lamely)
Us: (thinking) See Dick run. Run, Dick, run! See Jane sick. Heal, Jane, heal!
Us: (contemplating central line nurse's unmistakable irritation, and signaling frantically to Anni's nurse through the window) I'm sure we would have been told, if they were planning on placing another line.The line nurse charges out of the room, clearly disgruntled at having to deal with such high-maintenance parental patient baggage.The line nurse charges back into room, gathers supplies, and heads to correct room down the hall. Where the patient who actually needed the line was waiting.

12:42 p.m. One of the PICU residents stops by to let us know they'll be stopping Anni's anti-fungal.
Us: OK. And the liver team is also on board with this?
Her: Well, I don't know if we've talked to them. But this is really not a big deal. Even if she does have a fungal infection that we're not catching, since she has a normal immune system she should be able to fight off the infection herself with no problem.
Us: (a few blinks, wondering how to respond) OK, you mean normal except for the Prograf, Cellcept, and Prednisone? [ed.: common post-transplant immunosuppression drugs]
Her: (a few blinks) Oh. She's post-transplant?

3:37 p.m. Having heard that the parents in room 228 were difficult, Infectious Disease sends not one, not two, but three residents to explain the new plan of action to fight the suspected infection.
Them: We'll be stopping antibiotics X and Y, and replace them with antibiotic Z.
Us: OK. But antibiotic Z sounds like a Cephalosporin.
Them: (smiling in agreement) Yes! It is!Us: OK. But she's allergic to Cephalosporins. (picturing in our heads the warning stickers plastered all over her chart.)

Yes, all in one day! And no exaggeration, either, except I might be a bit off on the exact times. My memory's not that freakishly accurate.

But, still, we owe our daughter's life to her doctors and nurses at her hospital. We never forget that, and we know how wondrously talented they all are. One bad, scary, wretched day notwithstanding.

Nevertheless, medicine can be vertiginously, horrifyingly, gut-wrenchingly scary. Witness the following collection. I call it: Medical Links of Doom! (doom...doom...)"

"I couldn't tell you why for sure, when I was a youngster, though, my brother and I (especially during the time mother was gestating child number three) were rather dramatic. Were we trying to get (more) attention? Probably, yes.

One of the things we did often (besides put on skits about historical figures like George Washington and do commercials with household products for our parents) was to compile "fun fact to know and tell." Now, remember folks, this was prior to the Internet. This meant reading books and using paper and pencil.

Nowadays, it is so much easier. You post on your blog that you are hosting Pediatric Grand Rounds and people just send you fun facts to know and tell...! And so, in a tribute to my childhood (and perhaps yours!) here are some fun fact to know and tell..."

Most of them only needed some soft music and a story before drifting off, but there were a few troublemakers in the bunch. Eventually they all settled down and daddy can get a little sleep himself now. So sit back, prop your feet up, and enjoy the second installment of the Pediatric Grand Rounds."

"A popular saying in pediatrics is that children aren't just little adults. The wisdom of taking this to heart when approaching all aspects of pediatric health becomes clearer every day as I progress through my training. Kids are unique and have health issues every bit as interesting and complex as adults, and the people who raise, counsel, and treat them deserve a forum to discuss these issues. I had the idea to start a pediatric version of Grand Rounds a couple of weeks ago and I received some great submissions that are a solid foundation to build on. Here goes. "

In the tradition of Grand Rounds, a collection of medical blogging that is published every tuesday, Pediatric Grand Rounds is a way for those interested in pediatric health issues to interact and share their thoughts. This is not meant to be only for pediatricians however. Submissions are welcome from anyone interested in topics on the health of children.
